The following carriers are contracted directly with Doctors at the Personal Injury Clinic or through a third party and offer a contracted fee schedule. You may have only a co-pay or small percentage obligation depending on the details of each contract.

If you do not have Insurance benefits, other fees may apply to you for other srevices and procedures not covered by contract.

Initial Office visit and examination - fees depend on necessary diagnostic procedures.

Office visits are $50.

We follow the fee schedule recommended by the state of Utah for Automobile insurance carriers.
